Manchester Council has sent letters of apology to parents of dead children after they received school admission letters.
The parents of 95 children who have died were sent the correspondence, detailing how to enrol for school.
Manchester's Director of Children's Services, Paul Marshall, said the council "apologised unreservedly".
Mr Marshall said the council is reviewing their systems “to make sure this cannot happen again”.
