Nominations are now open for the Sovereign Healthcare Awards.
The awards, organised by the Bradford & District Disabled People's Forum and Sovereign Health Care, recognise those in the disabled community and the people who work with them.
A ceremony will be held as part of this year's Disability History Month event in December.
